Personally Identifiable Information (PII) is defined as:
- Any representation of information that permits the identity of an individual to whom the information applies to be reasonably inferred by either direct or indirect means.
Forms should never have fields that capture the following information:
- Personal identification numbers:
  - driver’s license
  - passport
  - social security number (SSN)
    - other government issued IDs (taxpayer identification number)
  - VIN number
  - financial account or credit card number
  - patient identification number
- Digital identity:
  - login name
  - screen name
  - nickname
  - handle
